Appointments may be scheduled during business hours by calling our direct line at (469)981-2648. The receptionist will inquire about the nature of your visit so that an appropriately urgent or routine visit may be arranged for you.
Same-day appointments, in-person or virtual, are available with our urgent care provider. These appointments may be requested by calling our direct line at (469) 981-2648.
Providing timely care is part of our commitment to providing the highest quality of care. If you are unable to keep an appointment, please contact our office promptly so that the time can be made available to other patients and your appointment can be rescheduled to a more convenient time. Failure to cancel appointments at least 24 hours in advance may result in a cancellation fee.
For needed medical assistance during office hours, we ask that, if possible, you contact the office at (469) 981-2648 before coming so that we can prepare for your needs. For medical assistance outside of regular office hours, please call (469) 981-2648 to leave a message for the provider on call. We will return your call as soon as possible. Please keep your telephone line open to receive the return call and remove the call block (if applicable).

A patient’s insurance card and a photo ID must be presented at each visit, and co-payments are due at the time of service. We will file claims with the insurance carrier as a service to the patient. The patient’s responsibility is to provide AMG with their current insurance card and notify us of any insurance changes. Deductibles or uncovered charges will be the patient’s responsibility and may be requested at the time of service. Patients with questions regarding their insurance coverage should contact their insurance carrier directly.
We accept fee assignment of Medicare claims; claims will be submitted directly to Medicare by our office. The portion of the fee not paid by Medicare will be billed to the patient or filed to secondary insurance.
Efforts are made to collect insurance deductibles, co-payments, and any other charges not covered by a patient’s insurance plan at the time of service. All accounts not collected at the time of service are due within 30 days from the date of service unless prior arrangements have been made. AMG reserves the right to request payment at the time of service at the discretion of Business Office personnel. New patients to the office who live out of area, are uninsured or carry a non-contracted insurance, will be asked to pay for their services at their appointment. Patients are responsible for payment of their accounts regardless of insurance coverage. AMG does not accept responsibility for collecting on a disputed claim. Delayed payment by an insurance carrier is not a valid reason for delayed payment on an account.
